At least seven incidents of some significance

The BBC:

the acting head of the HMRC said there had been seven incidents of “some significance” involving data security breaches since April 2005.

These “may well” indicate systemic failure, David Hartnett added.

The Register:

[Richard] Thomas [Information Commissioner] said that as a result of the HMRC scandal, “several departments have come to see us on a confessional basis, but none on anything like the scale of the HMRC loss”.

He said that Robert Hannigan, head of intelligence, security and resilience in the Cabinet Office, was continuing to review data handling by all government departments.
